• Your insurance card along with another form of I.D. (i.e. – Driver’s License, State ID card, Military ID)
• X-Rays, MRIs or other pertinent medical records
• Information about prescription medication which you may be taking
• Cash, Checks, Credit Cards (Visa, MasterCard, American Express, Discover)

 

Please fill out and bring the following to your appointment (to open the files, you'll need software like Adobe Reader):

1. AFAS Intake: Intake is available as a PDF file

3. Authorization from Patient or Legal Representative: Authorization/Patient or Representative PDF file
