What you should bring…
- Your insurance card along with another form of I.D. (i.e. – Driver’s License, State ID card, Military ID)
- X-Rays, MRIs or other pertinent medical records
- Information about prescription medication which you may be taking
- Cash, Checks, Credit Cards (Visa, MasterCard, American Express, Discover)
For your convenience our intake form is available as a PDF file that can be downloaded, printed and filled out prior to arriving for your appointment. To open the files, you'll need software like Adobe Reader or Foxit Reader.
